  VIOLATION: Observed unclean shelf liners on bar shelves, tears on bar stools and booth benches, padded bar top, unsealed wood
  in ladies rest room, and damaged floors behind bar. All surfaces must be smooth, durable, easily cleanable, and nonabsorbent.
   
  3717-1-04.0 Equipment, utensils, and linens: materials for construction and repair.
  (I)Nonfood-contact surfaces - materials.
  Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require
  frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.

  VIOLATION: Observed mouse droppings on kitchen shelf and under sink in ladies rest room. Clean and sanitize these areas to
  remove mouse droppings, and inspect facility on a regular basis. Contact pest control for additional assistance.
   
  3717-1-06.4 Physical facilities: maintenance and operation.
  (K)Controlling pests.
  The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises
  by:
      (1) Routinely inspecting incoming shipments of food and supplies;
      (2) Routinely inspecting the premises for evidence of pests;
      (3) Using methods, if pests are found, such as trapping devices or other means of pest control as specified
      under paragraphs (C), (K) and (L) of rule 3717-1-07.1 of the Administrative Code; and
      (4) Eliminating harborage conditions.

  VIOLATION: Observed dust and debris on extra glasses and counters at bar. THIS IS A CRITICAL VIOLATION.
   
  3717-1-04.5 Equipment, utensils, and linens: cleaning of equipment and utensils.
  (A)Equipment, food-contact surfaces, nonfood-contact surfaces, and utensils.
      (1) Equipment food-contact surfaces and utensils shall be clean to sight and touch.
      (2) The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits 
      and other soil accumulations.
      (3) Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, 
      and other debris.

  A plumbing system shall be:
      (1) Repaired according to the Ohio building code; and
      (2) Maintained in good repair.

  be provided.

  to keep them clean and maintained.
